All Seasons Sports Association Liability Waiver & Release Agreement
I understand that participation in sports and physical activities involves inherent risks, including but not limited to falls, collisions, contact with equipment or other participants, and other injuries that may occur during athletic activities.
I understand that All Seasons Sports provides developmentally appropriate, age-appropriate instruction in a supportive and non-competitive environment. However, participation in physical activities carries risks, and I voluntarily choose to allow my child to participate.
I agree to assume responsibility for the risks associated with my child’s participation and release All Seasons Sports, its owners, instructors, volunteers, and representatives from claims or liability arising from participation in the program, except where prohibited by law.
I confirm that my child is physically able to participate in All Seasons Sports activities and that I have provided accurate information regarding any allergies, medical conditions, injuries, or other concerns that may affect my child’s participation.
In the event of an emergency, I authorize All Seasons Sports staff to seek appropriate medical attention for my child if I cannot be reached immediately. I understand that reasonable efforts will be made to contact me or my emergency contact.
By submitting and signing the enrollment form, I acknowledge that I have read, understand, and agree to this Liability Waiver & Release Agreement and give permission for my child to participate in All Seasons Sports Association programs.
